Week 1

$200:  1 hour initial evaluation + education  on exercises and other self care techniques. You are given a comprehensive understanding of your condition and how to manage it independently. Therefore, you do not need a second visit in the first week.

$80:  $40 copay at initial evaluation.  You are instructed to schedule a second follow-up visit in the same week for which you again pay a $40 copay. Each visit may include only 10-15 minutes with your therapist, with the remaining time spent with an assistant, an aid, or independently doing unsupervised exercises. 

Week 2

$125:  60 minute 1-on-1 follow-up visit with your therapist, increasing your confidence and sense of empowerment to manage your condition. If appropriate, you are given explicit instruction on how to progress your exercises.

$80 :  2 follow-up visits x $40 copay each for only 10-15 minutes spent with your therapist. You may not get specific instruction on how to progress your plan.

Week 3

$0:  No visit needed due to  the quality time and education from your first 2 visits.

$80 :  2 follow-up visits x $40 copay each for only 10-15 minutes spent with your therapist.

Week 4

$125:  You are feeling 80-100% better and are confident in managing your condition independently. You have one final appointment for fine tuning and discharge.  

$80:  1 follow-up visit x $40 copay. For the second weekly visit, your insurance dictates that you need a re-evaluation ($40 copay) to determine continued therapy needs. You are feeling only about 50% better so your treatment plan is extended for an additional 4 weeks.

Weeks 5+

$0:  You are managing your condition independently and have the option to contact your therapist with questions, or to make a future appointment for a “tune-up” as needed. This option is typically not allowed with traditional insurance plans.

$320:  2 follow-up visits a week for 4 more weeks. If you are not feeling better after those initial 4 weeks, your insurance company may deny further coverage.
