What should I wear during physical therapy sessions?

You should wear comfortable clothing in which you can move freely and easily access your injured area.

How long will each visit last?

Visit duration depends on your injury and treatment needs, but you should plan for 45 minutes to 1 hour.

How many visits will i have?

Visit number depends on your individual treatment needs and your preferences. Because we are not controlled by insurance companies, we can meet for as many or as few sessions as you want. After your initial evaluation, you have the option to purchase treatment packages that will enable you to schedule appointments whenever you feel you need to touch base and get some guidance or to progress your exercise plan. 

Where is your office located?

EVOLVE Physical Therapy is a hybrid practice of mobile visits and telehealth visits.  This means we come to you either in person or via video! We can come to your home, your office, your gym — whatever is most convenient for you. Or, you can opt to do visits via telehealth, a popular option due to COVID 19.

Can I use my insurance?

EVOLVE Physical Therapy is a fee-for-service practice. This means that we are not contracted with any insurance  companies, but we can request out-of-network benefits for all insurance companies. You pay EVOLVE directly at time of service and then EVOLVE will submit a claim to your insurance company to access your out-of-network benefits. Each insurance company offers different out-of-network benefits.  You can call your insurance company directly to inquire about the specifics of your plan.

Can I use my HSA/FSA?

Yes, EVOLVE Physical Therapy can accept HSA/FSA for payment. However, certain accounts may have unique requirements. Contact your card issuer for specifics. Also, we are unable to accept partial payments from these accounts. The account must have a balance high enough to cover the full visit fee in order to be processed.  

What if I buy a package but don’t use all the visits?

You have options if you decide you don’t need to use all your visits.  You can apply the balance to a future service or transfer to a family member.  Or you can choose to be reimbursed the package fee minus the cost of the number of visits used at the per visit rate.
